Adult waiver

I certify that I am physically able to participate and will disclose relevant health conditions. I agree to follow studio rules and instructions. This waiver shall be governed by the laws of [State]. Minor waiver (parent/guardian)
Minor Participation Waiver & Consent I am the parent/guardian of the minor named below and consent to their participation in activities at [Studio Name]. I understand the risks involved and agree to release and hold harmless [Studio Name], its owners, instructors, and staff from any claims on behalf of the minor. I confirm the minor is able to participate and that I will inform staff of relevant medical conditions. I agree to emergency medical treatment as deemed necessary. This agreement is governed by the laws of [State].
